The Cause of (and Cure for) My Obesity

I have been rail thin ... and I have also carried more fat than I'd like to admit to. As a college wrestler, I was 4% body fat. In my late 20s, I ballooned to a whopping 30% body fat. I felt weak, tired, edgy, depressed, and was haunted by a constant craving for food - usually anything that had sugar. Disgusted with myself, I was determined to lose the unwanted fat while building more muscle ... in the shortest amount of time.

As an organic chemist trained in biochemistry, I scrutinized every available way to lose fat. I studied volumes of research on fad diets, the "food pyramid," and strenuous exercise programs. I mapped out the actions of purported fat-loss drugs like Wellbutrin, Phentermine, Xenical, Clenbuterol, Meridia, and amphetamines. What I learned was that these things could provide only temporary fat loss, at best, while putting my health at risk.

Digging deeper into science, I discovered that I could lose fat and gain solid muscle by adopting some simple habits that would help control a single hormone in my body: insulin!

Your body uses insulin to escort blood sugar and other nutrients into the muscle cells to be used for fuel. This keeps us alive and energized. However, too much insulin can be detrimental. It tells the body to store fat and, instead, use glucose for fuel. In addition, many hormonal systems that regulate appetite, mood, muscle growth, and even fertility are thrown out of whack by excess insulin. This state of imbalance leads to an addiction to sugar that has made the manufacturers of soda, juice, cereal, beer, and candy rich.

Most people have learned to pay attention to how many calories or grams of fat they put into their body. But they ignore the sugar. This is a deadly mistake. Low-calorie and low-fat foods tend to be loaded with sugar or "sugar mimics." These include sucrose, glucose, high-fructose corn syrup, monosodium glutamate, hydrolyzed proteins, trans-fat, and milk sugars such as lactose and maltose.

Looking back at my own eating habits, I see now that I was consuming sugar just about every time I put something into my mouth. Whether I was drinking a "sports drink," munching on a "health food" bar, or eating Campbell's soup, I was consuming some type of sugar. And that meant I was headed toward more treacherous health problems than just obesity.

Long-term, sugar consumption can turn this metabolic nightmare into a living hell. Habitually high insulin levels lead to a medical condition known as insulin resistance or Syndrome X.

The excess insulin numbs the muscle cells. Instead of allowing the glucose in so it can be used as fuel, the cells no longer react to it. Unable to gain entry into those cells, glucose remains in the bloodstream. Blood sugar skyrockets. Recognizing the rise in blood glucose, the pancreas produces even more insulin. Insulin and glucose overload leads to hypertension, heart disease, diabetes, and cancer. Aging accelerates so fast in people who suffer from insulin resistance that it can erase 10 years from their lives!

Alarmed by what I learned, I was now wildly motivated to control my insulin. And it wasn't difficult. I never went hungry. I never counted calories. I never suffered in the gym. I learned how to control my insulin by changing a few lifestyle habits. Doing so allowed me to burn fat day and night. My body innately knew when to eat and how much to eat.

By controlling my insulin, I descended from 30% body fat to a lean 12% body fat in 90 days. During that time, I gained six pounds of muscle. I had abundant energy all day. My mood was consistently upbeat. My physical and mental endurance was at an all-time high. My productivity quadrupled. With my newfound habits, I felt like I had found the Holy Grail of effortless fat loss. I awoke from my metabolic nightmare and freed myself from the impending health crisis that 80 million Americans are now faced with.

Here are the steps I took:

I learned to exercise properly. Proper exercise and insulin resistance cannot co-exist. I did interval training, which consisted of short bursts of intense exercise separated by short rest periods. And I worked out with weights 3 times per week. I attained the best results when I exercised first thing in the morning, before breakfast.

I quit sugar and sugar mimics. I scrutinized every food label to ensure that there were no sugars. If something tasted sweet, I didn't eat it - not even fruit for the first 90 days. I quit all soda and fruit juice and replaced it with purified water, water with squeezed lemon, or green tea. Instead of artificial sweeteners, I used only the natural sweetener Stevia.

I avoided dieting. Lowering food intake and dieting teaches the body to store fat. I ate a meal every 3-4 hours until I was totally full. My meals focused on healthy fat, vegetables, and protein. I ate grass-fed beef, eggs (the whole egg), pork, fish, whey isolate (free of sugar and artificial flavors), beans, chicken, organic salads (at least every other day), steamed vegetables, blueberries, coconut oil, organic bread with REAL butter, cashews, almonds, sunflower seeds, and avocados.

I got more sunshine. I exposed at least half of my body to sunshine for 15-20 minutes per day. There are numerous studies which show that sun exposure helps to normalize insulin and blood glucose levels. I also noticed that it helped control my appetite and mood.

I drank more water. Drinking water has been shown to increase thermogenesis by up to 30%, activating the body's natural ability to burn fat. I drank 16 ounces of purified water upon waking in the morning and 10 minutes before every meal.

I used supplements. As a drug chemist, I knew that nutritional supplements could be useful. After all, the vast majority of drugs are derived from natural nutritional sources. But it wasn't until I fully embraced them that I experienced their powerful benefits. To mobilize my fat and intensify my workouts, I used a thermogenic aid: a 30% extract from citrus aurantium mixed with other supportive herbs. To increase my insulin sensitivity, I used a 1% banaba extract with cinnamon and red ginseng. To help with digestion and circulation, I supplemented with cayenne pepper. To mitigate my intense sugar cravings, I used the essential amino acid L-tryptophan.

I cheated. In order to ensure that I could stick to my habits long-term, I cheated once a week. During this time, I did and ate whatever I wanted.

And I am thrilled to say that as other people heard about what I had done - and applied it - they had the same success, perhaps even greater. Consider Frank Dannenberg. At 5' 4", Frank weighed 205 pounds. He was a three-time heart attack victim and Type II diabetic. At a mere 58 years old, Frank was planning his own funeral. After following my recommendations for 90 days, Frank lost 23 pounds of fat, gained 10 pounds of muscle, and reversed his diabetes. With his newfound health, he also abandoned nine prescription drugs!

Without a doubt, the benefits of controlling insulin go beyond fat loss. Insulin control is the long-awaited panacea that Americans have been searching for. Physician David Katz, Director of the Prevention Research Center at Yale Medical School, summed it up best. We do recognize the fact that Moroccan Mint is traditionally served with sugar. We do not recommend adding flavoring agents to pure green teas however.

Do We Really Need Hepato-protectors?

There are some notorious high-selling ‘drugs’, classified in the Indonesian MIMS as hepato-protectors, while almost the rest of the world is devoid of it. It is also strange that they actually belong to dietary supplements (DS), a class of remedies that is by definition a non-drug (previously named ‘quasi drugs’) and therefore were without any controlled clinical trial to prove its efficacy as an effective medicine called drug. As we all know, the flood of supplements into our country (and the rest of the world) began in the mid 90's, made easy by a controversial Act of the United States called Dietary Supplement Health Education Act (DSHEA), 1993. This act had opened the door for all such remedies to enter the world, unproven by studies - normally required for all drugs to be marketed - or sometimes only ‘proven’ by mere cultural use. Therefore food supplements ( FS, a term used in Europe) ought not to be mixed in a compendium for drugs like MIMS, because doctors and pharmacists can’t differentiate unproven remedies from effective drugs. One could find a difference of registration code numbers. Regulation, in fact, requires that DS are clearly marked, but who bothers reading it or even understand the meaning? Therefore, the ‘education’ part of the Act should stipulate that the sale of FS must be coupled with adequate information to the public. Among others, it has to be clearly stated that the use of FS is no guarantee of a cure and that the user should decide for themselves whether it offers them a personal benefit. DS may also not claim a ‘disease claim’ on the label, but may only claim a ‘health claim’. This rule is often violated in developing countries, but the difference between disease and health may be subtle and debatable.

Isoprinosine™ (generic name, methisoprinol) was in the Indonesian market long before the above Act. When Isoprinosine’s registration dossier was assessed by the Indonesian National Drug Evaluation Committee it was unanimously rejected without hesitation. No proof of efficacy could be shown in the clinical trial study report done in the United States. These were widely publicized by the FDA and were known to us and therefore the fledgling was (prematurely) dead and the drug company was shut down (Isoprinosine was the only product of Newport Pharm. Int. Inc. USA, initially distributed by PT Wigo)

Despite of the negative recommendation of the Evaluation Committee, the Director General of POM issued a marketing approval under protest of the Committee. Even when the ‘drug’ was subsequently reevaluated because of registration renewal requirements of POM - every 5 years – and the change of ownership (acquisition of PT Dupa by PT Darya Varia) no new data were available and therefore marketing status should be withdrawn. However the ‘drug’ managed to stay happily in Indonesia until today with a ‘valid’ registration number.

Hepato-protectors (and Nootropics) are officially not included in the formal nomenclature of drug classes; it is not found in official books, obviously it has been newly created to suit a drug that doesn’t work. What should a hepato-protector do? Protect the liver against what and when, and for how long? Obviously, the name had its origin from animal studies, by which a chemical is experimented in rats whereby the compound is given (orally or parenterally) and followed by a challenger (usually a toxic compound). When it can be proven that the treated rats recover more favorably than the placebo-treated group, then the chemical could be said to protect the rat against a toxic stuff. If the primary outcome of the study happens to be observed in the liver, then it would be a perfect marketable name for a class of drugs to be called a liver-protector to trap the doctor into prescribing it. However, such an experiment can’t be found or is done in a human setting, because we can’t give a drug to a human for years to prevent a disease of the liver. It would be very unpredictable when that disease will be attacking the person (mostly none at all), and therefore if such a study should be done it would be impossible to perform. This would also be the difficulty to study a drug that could be proven to shorten wake-up time of a CNS depressant poisoning, for the simple reason that duration of a coma in a poisoned patient can’t be predicted from neither the dose of the poison or the depth of the coma. These two variables are all patient-specific, so that each person reacts in a haphazard manner when wake-up time is measured as the primary variable of the clinical trial. Simple observational, comparative outcome studies may bring more conclusive results. Such finding taught us not to give stimulants (amphetamines or caffeine) to poisoned patients in coma, resulting in dramatically reduced death rates from 25% to 2 % (1970’s) by just watching and caring.

Dozens of ineffective drugs like this have the lucky fate to have a long half-life in our country: Adona, ATP, Ketosteril, Neo-Minophagen, Fosfomycin IV, Anti-catarrhact eye drops, and many more. POM still refused to withdraw the drug(s) from the market, causing the truth being defeated by politics. This provokes for any quackery remedy to find their market share in our beloved country. Is it not reasonable then that patients go abroad for their flu? There is seemingly a lost of trust in our health system, inflicting hordes of people seeking refuge in other countries, a substantial loss for us, but a windfall of a few billion dollars annually to others’ benefit.

We will be waiting for our Health Minister to take immediate action, because doesn’t POM has the prime responsibility to protect the public from malicious drug-sellers? On top of all that, it also has created a stark conflict among doctors, an obvious reason for the organizers of this meeting to include this presentation with giving me a provocative title.

Let me close this writing with a fitting paraphrase from Shakespeare, an advice given by Polonius to his son Laertes, in Hamlet:

‘Those drugs thou hast, and their adoption tried,
Grapple them to thy soul with hoops of steel;
But do not dull thy palm with entertainment
Of each new-hatch’d unfledged remedy.’

Safety Update On Pioglitazone

Pioglitazone improves glycaemic control but it is unclear whether it improves diabetes–related clinical complications and mortality. Prescribers should consider this — along with recently emerging safety information — when assessing the ratio of potential harms and benefits for each patient.

Pioglitazone is a third–line choice for treatment of type 2 diabetes mellitus. It can be considered when either metformin or a sulfonylurea is contraindicated or not tolerated. Insulin should also be considered instead of pioglitazone in either situation.
Pioglitazone is currently approved as combination therapy in patients with type 2 diabetes that is inadequately controlled with insulin.
Pioglitazone is associated with weight gain, oedema and fluid retention and should not be used in patients with moderate to severe heart failure.

An analysis of data in a clinical trial database has found an increased rate of fractures of the arm, hand and lower leg among women using pioglitazone.
Doses should not be increased until after 8 weeks of treatment, as the full effect of the drug may not be seen before this time. In clinical trials of glitazone treatment, 25% to 30% of patients had no improvement in glycaemic control.

Homeopathy: The Test - Questions And Answers

How did homeopathy start?

Homeopathy was founded by Samuel Hahnemann (1755-1843). He trained as a physician, but thought that many of the techniques he learned were brutal and ineffective.

Disillusioned with medicine, he started work as a translator. It was then that he came across a book which mentioned that a bark extract, quinine, could be used to treat malaria. He found himself wondering why this worked.

Hahnemann started experimenting on himself. He noticed that when he took a dose of quinine he experienced feverish symptoms similar to those of malaria. This led him to formulate his universal rule for homeopathy: that like cures like.

Jacques Benveniste claimed he could explain homeopathy in the eighties. What happened to him?

Jacques Benveniste published a controversial paper on homeopathy in Nature in 1988. He implied that water had properties that meant that it 'remembered' what chemicals it had been in contact with. This results of this paper have since been called into question.

Following this incident, Benveniste lost his funding from the French government. However, he has continued his research with a small team and still stands by his original results.

His new research takes the concept of the memory of water a step further. He now claims to be able to record a signal stored in the water and turn it into a computer file, which can be emailed around the world. This emailed file can be played back into a sample of pure water, which then takes on the properties of the original substance.

These claims have met with even greater scepticism than his original results and have earned him an unprecedented second IgNobel prize.

How can the 'placebo effect' explain homeopathy in animals and babies?

Many people claim that homeopathy works simply because people believe it will. This is known as the placebo effect.

A major part of the placebo effect is the hope and peace-of-mind that you get from doing something you think will be beneficial. This requires the knowledge that the treatment is supposed to help you. Therefore the placebo effect should only work in humans old enough to know what a medicine is.

However, homeopathy is also believed to work on animals and babies. Could the placebo effect also explain this?

The apparent effect of a placebo could also be due to other interventions that occur at the same time - changes in diet for instance, or just increased care and attention. There could also be a degree of wishful thinking on behalf of the human observer - believing an animal or baby that received the treatment has improved more than it has because of unconscious bias. There might also be an indirect placebo effect - the treatment makes a carer feel more relaxed and this is picked up by an animal or baby.

Because of these possibilities, research (even on animals and babies) can only be convincing if it is 'double blind' and placebo controlled. This means that the researcher mustn't know which subjects have received the test treatment and which have received the placebo.

How has homeopathy performed in clinical trials?

There have been over 200 trials published that have examined the effectiveness of homeopathic medicines. The majority of these have found some positive effect of homeopathy. However, in such a comparison you have to take into account publication bias: a positive study is more likely to get published than a negative study. Opinions differ as to whether analysing all these studies together is useful and whether the overall evidence comes out significantly in favour of homeopathy.

The critics point to the lack of strong repetitions of studies. For instance David Reilly's work on allergy is often regarded as the best clinical evidence for homeopathy. However, in a recent attempt to investigate the same condition the results came out negative. Dr Reilly believes this is down to differences in the experimental method. Until a result can be reliably replicated in favour of homeopathy in independent laboratories the scientific community will remain sceptical.

What else would qualify for the Randi Million Dollar challenge?

The Randi challenge doesn't only apply to homeopathy, any paranormal effect would qualify. It started in 1964 when, during a heated radio debate, a parapsychologist challenged Randi to put his money where his mouth is. Randi replied by offering $10,000 of his own money and the Paranormal Challenge was born.

Since then, the prize fund has grown through donations and pledges by fellow sceptics to reach a total of more than $1m. To apply for the prize you just need to fill in a form on Randi's website.

So far, there have been applications from practitioners of therapeutic touch, dowsers and psychic readers. One recent application was from a 10-year-old Russian girl who claimed to be able to see using mental perception. When her mother blindfolded her she went on to successfully read out cards held up in front of her face. But when Randi applied the blindfolding, carefully making sure that there was no gap between the blindfold and her unusually concave nose, her mental perception deserted her. Needless to say, she failed the test.

Ceftriaxon Not To Be Combined With Calcium In Infusion

The revisions are based on new information that describes the potential risk associated with concomitant use of Rocephin with calcium or calcium containing solutions or products. Cases of fatal reactions with calcium-ceftriaxone precipitates in the lungs and kidneys in both term and premature neonates were reported. Hyperbilirubinemic neonates, especially prematures, should not be treated with Rocephin. The drug must not be mixed or administered simultaneously with calcium-containing solutions or products, even via different infusion lines. Additionally, calcium-containing solutions or products must not be administered within 48-hours of the last administration of ceftriaxone.

[Posted 05/22/2007] Novartis and FDA notified healthcare professionals of changes to the WARNINGS and ADVERSE REACTIONS sections of the product labeling for Exjade, a drug used to treat chronic iron overload due to blood transfusions (transfusional hemosiderosis) in patients 2 years of age and older. Cases of acute renal failure, some with a fatal outcome, have been reported following the post marketing use of Exjade. Most of the fatalities occurred in patients with multiple co-morbidities and who were in advanced stages of their hematological disorders. Additionally, there were post marketing reports of cytopenias, including agranulocytosis, neutropenia and thrombocytopenia in patients treated with Exjade where some of the patients died. The relationship of these episodes to treatment with Exjade is uncertain. Most of these patients had preexisting hematologic disorders that are frequently associated with bone marrow failure. Further, cases of leukocytoclastic vasculitis, urticaria, and hypersensitivity reactions (including anaphylaxis and angioedema) were reported.

Healthcare professionals should monitor serum creatinine in patients who are at increased risk of complications, having preexisting renal conditions, are elderly, have co-morbid conditions, or are receiving medicinal products that depress renal function. Blood counts should also be monitored regularly and treatment should be interrupted in patients who develop unexplained cytopenia.
